In traditional cognition, gout is a joint disease prevalent in the middle-aged and elderly population. The prevalence of gout increases with age, and the onset is obviously age-dependent. China’s gout epidemiological survey shows that the prevalence of gout in the 40 to 49 age group is highest in the male population; the prevalence of gout in the female population increases with age.
Why is the gout rate high in the elderly?
1. As the age increases, the glomerular filtration rate decreases. After 30 years of age, the glomerular filtration rate drops by 10 units every 10 years, which means that the ability of uric acid to clear and excrete in the kidney also decreases with age;
2. With the increase of age, the articular cartilage or synovial membrane of the middle-aged and elderly people gradually wears out, and the surface is no longer smooth, which is more conducive to the adhesion of urate crystals;
3. The social role of middle-aged men (40 to 49 years old) determines their high alcohol consumption and high frequency of drinking, coupled with the increased family and life pressure, making male gouts of this age group high; With the reduction of sex hormones, the prevalence of gout in elderly women is significantly increased with age.
But with the improvement of living habits and living standards, things are changing: the prevalence of gout in the middle-aged and elderly population is not decreasing, and more and more young people are beginning to gout.
Why are more and more young people starting to gout?
Gout is a multi-gene hereditary disease, and genetic factors are the main cause of gout. The genetic susceptibility genes of gout have been lurking in these young people. Under some incentives (regular diet, irregular work, lack of exercise and obesity, etc.), the balance in the human body is broken, and the genetic susceptibility gene of gout is broken. Without the corresponding restrictions, it began to wreak havoc. In other words, most gout young people should have a family history of gout (both explicit and recessive), with a positive rate of more than 70%. In addition, gout secondary to other diseases is also relatively common among young gout populations.
I understand that gout is a genetic disease. It is not an accident if gout is not sticky. In addition, only 20% of the uric acid in the human body is eaten, and another 80% is derived from the metabolism of its own cells. The diet can only change the amount of uric acid produced by the exogenous food, and there is no uric acid produced by the metabolism of its own cells. It is less likely to change the genetic susceptibility of gout. Another point to note is that wine is a well-known sorghum food, but some foods with very low strontium will also increase blood uric acid and even induce gout. These foods include carbonated drinks, sugary drinks, etc., which are very popular among young people.
For young gout susceptible people, although the genetics can not be changed, it is a feasible and effective means of gout prevention to actively change the original living habits and make them healthier. Compared with adult patients, young gout patients tend to have higher blood uric acid levels, gout conditions are more serious, and kidney involvement is more likely to occur, so there is a need for standardized urate-lowering therapy.
